Real-time intraoperative monitoring of myocardial ischemia in noncardiac surgery.

چکیده

FOR the past 15 yr, there has been intense interest in the anesthesia community in the detection of intraand postoperative myocardial ischemia as a means of optimizing patient care because of the accumulated evidence suggesting a strong association with perioperative cardiac morbidity and long-term outcome. ',*It is important to understand the limitations of the methods of detecting myocardial ischemia. This review will focus on understanding the current state of real-time intraoperative ischemia monitoring, while the reader is referred to other articles that highlight the importance of emergence and postoperative myocardial ischemia. 3,4 Although this review focuses on noncardiac surgery, many of the data were obtained from the cardiac surgical patient because of the lack of studies in the noncardiac surgical population. It is thought that data from patients undergoing cardiac surgery may be applied to those undergoing other surgical procedures.

Risk stratification for noncardiac surgery. How (and why)?

A lthough the occurrence of life-threatening cardiac complications during and after noncardiac surgery has long been recognized, this problem has gained increasing attention over the past 15 years. In part, this new interest reflects the growing numbers of complex operations in an aging population and increased scrutiny of what were once considered expected events. However, it is also clear tha...

A comparison of methods for the detection of myocardial ischemia during noncardiac surgery: automated ST-segment analysis systems, electrocardiography, and transesophageal echocardiography.

Clinicians often fail to detect intraoperative ischemic electrocardiographic (ECG) changes when viewing oscilloscopes. Automated ST-segment monitors promise to increase the detection of such ECG changes. We investigated the capacity of two commercially available ST-segment monitors to detect intraoperative myocardial ischemia in patients at high risk for developing intraoperative myocardial isc...

Myocardial ischemia--association with perioperative cardiac morbidity.

The development of ambulatory electrocardiographic recorders and analysers and the application of transesophageal echocardiography in the mid-1980's enabled investigators to quantify and describe the occurrence of silent as well as symptomatic ischemia in the perioperative period. Several technical advances which have recently occurred in ECG monitoring include the use of miniaturized digital c...

The usefulness of transesophageal echocardiography during intraoperative cardiac arrest in noncardiac surgery.

According to guidelines established by the American Society of Anesthesiologists and the Society of Cardiovascular Anesthesiologists, life-threatening hemodynamic disturbances are classified as a category I indication for the intraoperative use of transesophageal echocardiography (TEE). However, the usefulness of TEE during intraoperative cardiac arrest and its impact on patient management have...

Myocardial injury after noncardiac surgery.

PURPOSE OF REVIEW Recent investigations have substantially improved our understanding of myocardial injury after noncardiac surgery (MINS). RECENT FINDINGS MINS is defined as a prognostically relevant myocardial injury due to ischemia that occurs during or within 30 days after noncardiac surgery. MINS occurs in 8% of adults undergoing major noncardiac surgery and is diagnosed with an elevated...
